- Proof of Nevada Business Registration
- Clark County / Local Jurisdiction Registration (if applicable)
3. Insurance Requirements
(Must name Dignity Fire Protection, Inc. as Certificate Holder & Additional Insured)
- General Liability Insurance
- Minimum: $1,000,000 per occurrence
- Workers’ Compensation Insurance
- Required for all contractors with employees
- Automobile Liability Insurance
- Required if company vehicles are used
- Professional Liability / Errors & Omissions (if applicable)
- Current Certificate of Insurance (COI)
4. Employee & Workforce Documentation
- List of Authorized Employees / Technicians (if required)
- Proof of OSHA 10 or OSHA 30 Certification (if required)
- Fire sprinkler or alarm NICET Certification (if required by scope)
- Proof of Background Check (if required)
- Valid Government-Issued Photo ID
5. Safety & Compliance
- Signed Safety Policy Acknowledgement (if required)
- Job-site Safety Plan / JHA (Job Hazard Analysis) ( if required)
- Proof of Drug-Free Workplace Policy (if required)
- Agreement to comply with NFPA, AHJ, and Local Fire Codes
6. Financial & Administrative
- Check for Payment information
- Invoice submission form & billing contact
- Lien Waiver (Conditional & Final, as required per project)
- Payment schedule acknowledgment
7. Project-Specific Requirements (As Applicable)
- Shop drawings approval
- Permits & inspection sign-offs
- Manufacturer certifications
- As-built drawings
- Testing & commissioning reports
8. Acknowledgements
- Agreement to comply with Dignity Fire Protection, Inc. policies
- Agreement to maintain active insurance and licensing
- Agreement to notify Dignity Fire Protection of any changes immediately
